Spanish Speakers

Core hospice services are covered–nurse, social worker, and chaplain are fluent in Spanish.

Hospice Aides (CNAs)

Hospice Aides can be assigned up to 6 days per week for home maker along with CNA tasks. We value aging in place.

 

Clinical Team

Clinical team of hospice professionals specifically trained in end of life care and its challenges.

Counselors

Masters level counselors on staff.

Thoughtful Transitions Project

A cost-free community resource to offer Transitions Coaching/Counseling with the goal of making residents and their families feel safe and comfortable as they move through losses in life that occur when relocating.

Care Plans

Individualized care plans to meet each patient’s needs with a focus on pain control and symptom management.

RN follow-up visits

24 and 48 hour RN follow-up visits are made without exception when altering a scheduled pain medication.

Continuity of Care

Continuity of care – patients are able to develop a sense of comfort and peace because treatment is provided by the same team of caregivers.

Visiting Physician

Physicians make in home visits to help diagnose and consult chronic patients that may be difficult to qualify for hospice.

Dedicated Evening & Weekend Nursing Team

This team starts their day when yours is ending.

Patient/Caregiver Ratios

Low patient to caregiver ratios – allows caregivers to take their time and provide thorough, complete care.

 

Visiting Nurses

All our visiting nurses are at an RN level or above and All of our patients receive a minimum of 2 RN visits per week.

Involvement Encouraged

Patient and family involvement are highly encouraged.

24/7 On-Call

On call nurses and psychosocial team available 24 hours a day, 7 days a week for questions and visits.

“Happiness Is”

“Happiness Is” program – helps fulfill meaningful last requests, improving quality of life and helping families make memories together.

Spiritual Care

Spiritual care team serves all faith practices with specialized training in Protestant and Catholic theology and rituals.

Music Therapy

Music therapy – clinical and evidence based use of music to address physical, emotional, cognitive and social needs of individuals by a board certified music therapist.

Other Therapies

Other alternative therapies include pet and massage therapy.

Community Approach

Active in hospice community – members of CAPC and MHPCA.
